I hereby authorize LEAP Global Missions to investigate my background and qualifications for purposes of credentialing requirements for an upcoming LEAP mission trip.

I understand that LEAP will utilize an outside firm or firms to assist in checking such information, and I specifically authorize such an investigation by information services and outside entities of the organization's choice.

I understand that this release form is valid for three (3) years, at which time I will be asked to submit a new release form.

I also understand that I may withhold my permission and that in such a case, no investigation will be done, and I will not be able to participate on the mission.
